If your child is 17 years old or younger or you are 13 years old or older, you may now go online to request a MyChart account. Simply select the form that best describes you. A valid photo ID is required to signup online, so look for the secure upload to submit your proof of identification.
You can signup for MyChart at any Cincinnati Children’s clinic at any time (Burnet Campus, Liberty Campus or any neighborhood location). No appointment is necessary.
From the MyChart login screen on the MyChart website or MyChart mobile app, select SignUp Now. Enter your activation code and other personal verification items, such as the last four digits of your Social Security number and your date of birth. Select Next.
